ALS Existing Facility User - Coming Onsite

This process is intended for:

• ALS Users who currently have active appointments at Berkeley Lab, and
• Do not have Berkeley Lab login credentials and therefore cannot log into the Lab's Vaccination portal at clinic.lbl.gov, and
• Will be arriving at the ALS in the near future and need to complete a COVID Questionnaire and upload a copy of the vaccination documentation (if applicable)

We are using the Lab's applicant tracking system to facilitate the collection of this questionnaire/card upload. The process using this tool is:

• The user will complete this initial information in order to provide HR with basic information
• Once HR has been notified that the basic information has been provided, they will initiate the COVID documentation process
• The user will receive an email asking them to complete the COVID questionnaire and upload a copy of the COVID card, if applicable
• Once submitted, the system will send an email to the user and the ALS User Office identifying which safety protocol is applicable (A or B). Visit ehs.lbl.gov/coronavirus to review safety protocols in more detail. 

Since we are expanding the use of an existing system to collect this information, you will likely see questions that do not apply to active users.

• Please DO provide any required fields including updated contact, address, and citizenship information
• For expected start and end date, please indicate the dates you expect to be onsite next
• You DO NOT need to provide detailed education, financial, or sponsoring institution information, as that is already on file. For those questions, feel free to add N/A to any required field.
• It's recommended that you use the same email address that you're using on the user portal. The system will indicate if you already have a profile, and you can opt to reset your password if you do not have that available.
